S1 Form Moving to Spain From UK: Pensioner Checklist

Published by Expat Compass Checked against official sources: 19 October 2026

An S1 form can fund state healthcare after moving to Spain from the UK, but holding a GHIC or receiving a State Pension does not complete Spanish registration. The costly mistake is assuming that an S1 request in progress has already created a route into the system.

This is for someone moving to Spain who may be eligible for UK-funded healthcare through a UK State Pension or qualifying exportable benefit. It does not decide residence, visa, family-dependant or insurance eligibility.

1. Check whether an S1 route actually fits before relying on it

An S1 is a certificate showing that one country is responsible for the holder's state healthcare costs while they live in another. For a UK-issued S1 used in Spain, the UK must first confirm that it is responsible for your cover.

GOV.UK says that people with a UK State Pension or another qualifying exportable benefit can request an application form from NHS Overseas Healthcare Services. Some posted workers, frontier workers and eligible dependants can also have an S1 route, but their facts and application route are different. A move to Spain, a British passport or a retirement plan alone does not establish eligibility.

Start by identifying the route you think applies: State Pension, qualifying benefit, UK employment, cross-border work or dependency on another person. Then ask the issuing body to confirm the route, rather than sending a general form in the hope that it covers every case. HMRC's CA8454, for example, is for specified dependants and people on maternity, paternity or adoption leave; it is not the standard pensioner application.

If you are still deciding when to claim a UK State Pension, use the State Pension forecast modeller to organise your current forecast, proposed start date and income assumptions. It cannot confirm an S1 entitlement or replace the official pension forecast and Overseas Healthcare Services decision.

2. Separate the S1 decision from Spanish residence paperwork

Spain requires people staying for more than three months to register as resident or use the appropriate visa route. GOV.UK says you normally need to show healthcare cover for that process. Do not book appointments on the assumption that an S1 application, rather than an issued and registered S1, is proof that the healthcare step is complete.

For people planning later life in Spain, GOV.UK lists a valid British passport, residence document and municipal registration document (the padrón) among the documents needed to register for state healthcare. Local practice and the documents requested can change, so check the national and regional instructions for the place where you will live before travelling for an appointment.

Put the dates for residence, padrón, S1 issue and health registration in the Moving Abroad Action Plan. It organises the dependency order but cannot book an INSS appointment, determine immigration status or make an S1 valid.

3. Request the S1 and keep a complete evidence pack

Request the form through the body responsible for your route. A pensioner or qualifying-benefit route goes through NHS Overseas Healthcare Services; work-based routes can involve HMRC social-security documentation. Keep copies of the request, the issue decision and the final S1, not just a call note or a screenshot.

Before you move, assemble:

  • your passport and Spanish residence documents or the evidence required for them;
  • your padrón certificate once you have registered with the local town hall;
  • State Pension or qualifying-benefit evidence, if that is your route;
  • the original S1 and correspondence showing who issued it;
  • your Spanish address, NIE and contact details where available;
  • documents proving relationships for any family member whose status needs separate consideration; and
  • a short medical summary, prescription list and travel or interim-insurance policy documents.

Family members are not automatically covered merely because they travel with the main holder. GOV.UK notes that dependants and family members are classified differently in Spain than in the UK. Ask the INSS which documents it needs for each person, and keep the answer with the application record.

4. Register the issued S1 with INSS, then register locally

An S1 that has not been registered in Spain is not the end of the process. GOV.UK directs S1 holders to register it with the Instituto Nacional de la Seguridad Social (INSS), either through its online portal or at a local INSS office. The Spanish Social Security service publishes a specific form for registering an S1 for healthcare cover in Spain; its document list includes the S1, identification and, where relevant, relationship evidence.

Once INSS processes the registration, GOV.UK says it sends a Spanish social-security number by post. Take that number to your local health centre to register and obtain the medical card used for doctor, hospital and pharmacy access. Keep each confirmation: the S1 issue notice, INSS registration evidence, social-security number and health-centre card are separate steps with different purposes.

If registration is delayed and you need emergency or urgent treatment, GOV.UK says to contact NHS Overseas Healthcare Services. An administrative delay is not proof of permanent cover.

5. Plan healthcare for the period before registration is confirmed

A GHIC or EHIC is for medically necessary state healthcare during an eligible temporary stay; it is not a substitute for establishing resident cover. The Spanish Social Security service likewise describes its European Health Insurance Card as a document for temporary stays, not for a move of residence.

Work out the realistic gap between arriving in Spain and completing the local process. Check whether your policy covers pre-existing conditions, outpatient care, private facilities, repatriation and any excess, rather than comparing only the premium. If you are working or self-employed in Spain, state healthcare can instead follow Spanish social-security contributions; that is a different route to confirm with the Spanish authority.

Use the free health insurance comparison guide to make a question list from your destination, household, existing conditions, network preference and expected gap. The healthcare cost estimator can then model a planning range. Neither tool quotes an insurer, confirms underwriting or decides entitlement to Spanish state healthcare.

When professional help is worth arranging

Get written confirmation before making a significant residence, insurance or treatment decision if you have a UK and Spanish work history, are relying on an exportable benefit, have dependants, are changing country during the year, need ongoing specialist treatment or have a material insurance exclusion. An immigration professional can advise on the residence route; an authorised insurance adviser can explain policy suitability. The NHS, HMRC, INSS and regional health authority remain the sources for their own decisions.

Guidance, not advice: ExpatCompass tools organise dates, records and budget questions. They do not decide S1 eligibility, Spanish residence, family-member status, insurer acceptance or the healthcare you will receive.

A sensible S1 registration order for Spain

  1. Confirm which healthcare route applies to you before relying on a GHIC or an S1 request.
  2. Obtain the issued S1 through the responsible UK body and save every supporting document.
  3. Complete the Spanish residence and padrón steps required for your situation.
  4. Register the issued S1 with INSS online or through the appropriate office.
  5. Keep the INSS confirmation and use the resulting Spanish social-security number at the local health centre.
  6. Keep suitable interim cover until your own registration route is confirmed.
  7. Recheck the position after a pension, work, address or family-status change.

The useful outcome is not simply holding an S1. It is a documented, registered route into the Spanish system, with a plan for the period before it is active.
